Thanks for reaching out to us! We should be able to help with this. However, I'm afraid your top fitting is too damaged to ID. Can you show us an undamaged version? You could use the balance on the other side of the window as a reference.
Thank you very much! I'm sorry, I thought the top fitting was broken off. This reminds me of the 19-011. Take a look at the specifications and see what you think. The 19-011 top fitting isn't used with our stock balances but we can still help via custom order.
Can you measure the bottom shoe as shown below and tell us what you get? Can you also confirm the length of the metal channel? Unfortunately, the 2190 stamp is outdated, so we will need you to weigh the window sash on a scale and tell us the exact weight.

On the top bracket, those 19-011 measurements match well.
The metal channel is 22 inches long and the channel edges are hammed (folded over); see my earlier photos.
On the shoe bracket, it also matches the 15-001 at 1"+1/8" wide.
So I think that would work well. We could also replace the window balances on both sides if that created any concerns. The window sash weighs 24 pounds.

Thanks for getting back to me, Pat! I'll go ahead and email you a purchase link for (2) 22" balances with the 19-011 top fitting, 15-001 bottom shoe, and 2150 stamp. A pair of balances with this stamp will support a sash weighing 23 to 27 pounds.
Our metal channel isn't folded over on the sides like yours but that won't affect function. Please let me know if you have any questions!
