Hey, thanks for posting. I've narrowed this down to two possible channel balance Series: either our Series 380 or our Series 385 balances.
Now, as you can see, there is only one difference between these two Series, and that is their bottom shoe. The Series 380 uses the 15-005 while the Series 385 uses the 15-004. I can't quite tell what you have based on your pictures, though. Please carefully compare their dimensions to your own hardware and see which is a better match.

The length of the bottom shoe of my channel balance is a little greater than 1 inch so I think I will go with the series 385/15-004. Which stamp option should I choose?

I agree! If it's larger than 1", then you likely have a 15-004 that's worn down. As for the option, based on your stamp I'd say that you go with the 2120 option. You might want to weigh your window sash just to make sure that's right.

I just ordered this channel balance. It showed that I ordered 385-22. Is this correct?

Well, you said your metal channel is 22", so that's correct. And since you said you had the 15-004, and I also believe you have the 19-005 top, then Series 385 looks right, as well.
