Thanks for reaching out to us, John. We don't sell this exact operator, unfortunately. Our 39-089 and 39-090 are our best options but they lack the offset yours has so we can't promise they'll work. Take a look and see what you think. 

As for the gasket and screws, see the 39-303 and 39-083 which are sold separately.
