Truth 45352, number on arm is 31876. I removed the operator on the left in the image but included pics of the right one for reference as it is still operational. The case is 4"7/8, the arm extended from the case is about 9"?

The handling of the operator is determined by the side the handle attaches to. The 39-220B has the spline mounted on the left side of the base, so it would use the 39-452 recommended above. If you need the 39-221B with a right-mounted spline as well, see the 39-451.
