Hello, Brian. I'm afraid I don't know anything about your door, and with the hardware missing it's going to be next to impossible for us to know what you have. 

The best place to start would be with two key details: the thickness of the door, and the distance between the two outermost installation holes, center-to-center. Ignore the middle hole. With that data I might be able to find something suitable, but please understand that at best it's going to be a shot in the dark. The only thing I know about your door right now is that it's missing a handle and it looks pretty old.
