Thanks for posting! Your window operator reminds me of the 39-253RB Encore Dual Arm Operator. This is used along with the 39-529 Truth Hardware Track Slider, which travels is the 39-528 14-1/2" Casement Track. You'll need the 39-376 to replace the cover. (The opposite versions of the operator and cover kit can be found tagged below.)

Be sure to look over the dimensions and double check them to confirm this is what you need for the window.
