Issues with crank operators

A home owner from San Diego says:
Hi! I have crank window operators and I have three different issues. The first is that the end of the operators are coming out of the slider attachment that is on the window itself. There must be a stopper that I am missing that is causing this. The second is that the slider attachment is broken on one of the windows. The third issue is that the locking mechanism on one of my windows is bent. I can't find any information on any of the hardware that tells the manufacturer. There is a marking on the glass that says Cardinal. Any help you can give is much appreciated.

Dave Sr. from SWISCO responded:

Your operator looks similar to our 39-433. All you should need is the 39-433B bracket. I believe the bracket is held on with friction. As for the lock, take a look at the 38-205 and the 38-208. I can't get a good look at the lock you now have, but review these parts and see how they match up.
