It is a 3240

Question

The black piece has a tab broke of one side. The channel is 1/2 in wide by 33in long. Cannot identify which top piece will work

Start by looking at our S385-33 channel balance. This comes with the 19-005 top and 15-004 bottom, so as long as these match you should be okay. I would also recommend the 3240 option, of course.
