I looked at the one you have but the two lower holes were not 1 1/8" it more like
1 1/2" apart. I hope the pictures will help. Notice the 1st picture had numbers not sure if you can cross reference these numbers.

It's tough to make out what you have from these pictures. I can't make out the top hole pattern and I'm also not sure what your dimensions are, other than that the two lower holes are 1-1/2" apart. The closest match I can find is our 39-258. Could this be what you're looking for?
