I have a Jeld Wen window balance I need. See attached pics. Unique 26.5-D-40. 1/2" wide 28" total length.

Hmm...I am having a little difficulty seeing the ends and the length of the balance in this photo. I have known Jeld Wen windows to use our Series 385 channel balance that includes a 19-005 top and 15-004 bottom balance shoes. I recommend you taking a look and comparing your balance to ours to see if this series is the correct one for you.
