Hi Tom,
Thanks for reaching out to us! This Jeld-Wen lock resembles either the 38-095 or 38-096 Maxim Multi-Point Locks. Although, to confirm which of these might be the right replacement, you'll need to look over the diagrams below and compare them.
They each have the same measurement for the distance between the installation holes.
