It looks like your worm gear stripped out, so you will need an all new operator. I'm just not sure what to recommend; your photos only show the base, which only tells half the story. We have to see the entire mechanism in order to make a recommendation.
Your cover is cracked, so you'll need that too. Our 39-107 should work perfectly well. I just need to see a picture that shows the entire operator mechanism so that we can get that to you.
