cannot find this pivot bar with 3 bolt holes. it 2 inches long and the tip coming out to the shoe is a 1/4 where it goes into the shoe, it has numbers 6 2 46 on it.

Don't worry about the number of holes. Your sash only uses one hole, and the position for the one that matters matches our 26-327, where the rear hole to tip distance is 1-9/16".
