This appears to be either a Series 380 or Series 385 Channel Balance. The onyl difference between these two balances is the depth of the bottom nylon shoe, or the 'heel-to-toe' measurement, so compare both with your old hardware to see what matches best. 

In either case, you'll need to measure the metal channel length to determine the length needed; don't include the nylon fittings in that measurement. Finally, you'll have to choose a stamp option. Look for a stamp code on the metal channel that indicates the option needed. If there is none or you're confused, obtain the exact weight of the window sash. Then match that exact weight with one of the weight ranges listed alongside each stamp option under the balance's product description. 

For the Top Sash Guide, Kinco windows often use the 18-166 but sometimes the 18-188 as well. Compare both with your old guides to see what matches up.
