Hello there!

Our 40-150 comes to mind though I can't be certain since your hardware is damaged. I'm afraid we don't sell the base plate by itself though. Look over the specifications and see what you think.

I still have handle and square shaft, do I just need screws to fix?

These handles are "pressed" on at the factory. Once they come apart, there is no re-attaching them back together. I have noticed your latch has a locking lever on the base of the inside handle but note that the 40-150 does not.
