Hey there,
The worm gear has become stripped from the base of the operator, so you'll need to replace the entire part. You'll need the 39-031 EntryGard Dual Arm Operators to replace it. I'm confident this should be what you need but be sure to take a look over the specifications and compare them to confirm.

Please see the tutorial below for instructions on how to remove/install the hardware. You will likely need to remove the wood trim for the installation. There are a few others tagged below this thread as well.
