Lever arms?

Your operator resembles one of the Encore pairs shown in our video below. The determining factor would be the fitting used to attach the straight arm to the sash. Both sets are used with the 39-375 and 39-375 handles with covers. 

As for the hinges, I can't make out all of the details with the sash and screen still installed but the 36-048 would be a good place to start. Review the images and installation video to see how they compare to yours, paying close attention to the 10" long base attached to the window sill.
