Looking to replace this door handle I purchased more than 20 years ago.

It appears that your handle set has a centered thumb lever. I recommend reviewing the 82-030A keyed handle set that has a 3-15/16" installation hole spacing from center to center. Review the overall dimensions to confirm a good fit and let us know what you think!
