This looks like an old Andersen window operator. Yours isn't made anymore, but you might be able to use our newer 39-094 and 39-095, but I cannot but sure that these will work as well as the old one. You will likely need to adapt the sill trim to accommodate the new style base. 

Sorry that we can't offer a better replacement. If you do try the 39-094 and 39-095 out, I'd love to hear how it went.
