Looking for window balance information

I'm trying to find out more about this window balance and where I can get it. This was in a vinyl window, I believe it is a tilt, and the stamp on the metal says 27D. Someone told me this might be a PlyGem window balance.

The window balance you have here is the S795-27 Channel Balance. Although, yours has a hook attached to the top of it, so I'll need to see just one more photo to determine if this might need to be a custom assembly.

May I see a photo of the installation hole at the top of the window track?
