The part that is needed is to replace two broken lower pivot bars - the pic I sent is a broken bar that I taped but is broken between the two mounting holes
I took two more pictures one of the shoe from the broken window and one from a shoe with a window with intact pivot bars. Hopefully this helps.

Unforutnately, we were not able to locate a match for your offset pivot bar. With that being said, you may be able to substitute your broken pivot bar with a straight bar without compromising the window sash performance. Take a look and review the specifications of the 26-422 pivot bar shown below.
