Thanks for posting! For the operator, please take a look at our 39-139 Single Arm Crank Operator and compare it to your own hardware. Let me know if you think that will work.
For the lock, I have a few options I'd like you to look at. Please see our 38-095 Maxim Multi-Point Lock, our 38-138 Multi-Point Lock with a 29MM Fork, and a 38-139 Multi-Point Lock with a 37MM fork. Do any of these match your hardware?

Do you have a version of the 39-138/9 that is rear mounted?

We do not currently have a rear-mounted version of this operator, but I'd be happy to research something for you. Before I can recommend anything, however, I would just like you to confirm the length of your arm.
When measuring this length, make sure that you're only measuring from the center of the wheel at the end of the arm to the center of the gear at the base. Let me know!
