the gear is worn and won't open all the way anymore, we need a replacement

We do carry the 39-379 skylight operator, but I'm not sure if it will work for your window. Take a look at it and see if you think it's a possibility.

Also from Philadelphia, and am having the same exact issues at this current moment.

The dimensions of the piece in the photo are
11" long
1 7/8 H
(not including height from chain exit)
3/4" - 2 3/8" D.

I believe the truth would work fine if the height wasn't a factor. Is there an operator with that can fit the request?

I'm sorry to say that the 39-379 is currently the only skylight operator we sell. If its dimensions will not fit your application, then we unfortunately have no alternative.
