Have you seen our 81-246 Roller Assembly? This appears to be similar to your old hardware. Please compare the dimensions on our product page and let me know if you think it'll work as a substitute. Thanks for posting!

I ordered the 23-218 version and I'm waiting to receive and try it out.

Let us know how it goes. The 23-218 is normally used for sliding closet doors so my main concern would be the weight difference using this in a patio door. That being said, if the dimensions match up to your original roller, it could work for you.
