Looking inside the cup with the curve on the bottom, in the center it says Grass. The number at the upper left is 850 with 01 under it. The number in the upper right is 97-11. Across the bottom the patient # is RE 34.995

Unfortunately, we do not carry an exact match for your Grass hinge assembly but take a look and compare the specifications of the 32-345 to see if you could adapt this to replace your original hinge. Thanks for posting!
