It has "Miami window Corp, part number 2383912" cast on it. It has a longer crank compared to your 39-054. There is a tile wall next to and below the window.

What you have here is an "over the sill" style operator, which is no longer available. I think you should look at the 39-054 operator again, along with a 39-422 extension. See if you can use this set up for your window. My hope is that the 39-422 extension, or even two of them, could give you the clearance you need.
