It looks like you may have the S390-27 channel balance. This comes attached with the 19-002 top and 15-001 1-1/8" deep fitting so you should be good to go as long as your specifications match up with this. 

Note, the stamp on your metal channel is outdated so it doesn't match any of the strength options we have. With that said, you'll need to weigh your sash and refer to the chart under the product description on the S390 store page to determine the appropriate spring strength option for your balance.
