Thanks for posting. I believe you have either our S380-27 or S385-27 Channel Balance. The only difference between these two balances is the depth of the bottom nylon shoe, or the 'heel-to-toe' measurement, so compare both with your old hardware to see what matches best. See the diagram below for reference. In either case, choose 2640 as the stamp option.

For your Top Sash Guide, we've seen this one before and have never been able to find an exact match. We suspect our 18-009 might be your best bet, but review the dimensions below and see how they compare to an undamaged guide. Let me know what you find.
