Milgard Aluminum single hung window balances

A quick learner from California, Bay Area says:

I have some broken balances for these windows. I would like to identify them and place an order. The marking on the existing balance says "26 4 Caldwell HM". I am also attaching some pics. Please let me know which part number on your website I need to order.
