That looks like an older version of the new Encore style 39-221B. I believe this is the operator you're looking for. Please note that the cover and handle are now sold separately under the 39-451.

See the 39-220B operator and 39-452 handle/cover for the opposite side. You can reuse your old bracket, but just in case you can't, see our 39-546 and 39-545.
