The operator shown in your first and last photos resembles one of the dual-arm operator pairs shown in our video below, depending on the fitting attached to the end of the straight arm.

With that being said, your second photo only shows an operator with a split and and no straight arm. If that is the case, take a look at the 39-220B and 39-221B. Review the measurements below to confirm they match your original assembly. Thanks for posting!
