Milgard single hing sash balancers

Question

Hi, I would like to buy a pair of balancers for a 36x48 Milgard single hung sash window (operable section 29 3/8" x 19 1/8" viewable).

Stamped ID # is: "19 5 BSI TL".

Length of the metal section is 19 3/8", 9/16" wide (1/2"), 5/8" depth.
Lower guide pads are 15/16" wide, depth of guide is 1 1/16".
Upper guide pads are 15/16" wide, depth of guide is 11/16".

Window is 4-5 years old.

One of the metal hooks broke on one balance, I suspect from overloading due to improper maintenance and lubrication. I would like a new pair of balances if you have them.
