Milgard window balance

Question

I have 6 no-tilt sash windows that are hard to open and close?. One certainly has a bad balance as it doesn't retract properly etc. Others have various levels of effort required, none easy.

All 6 Milgard windows are identical, opening width/height 33.5"W 70"T sash 33.5"w 35"T

Photos attached ?.. Stamping says "31 4 BSI TF" Metal length is 31.5 and has plastic shoes on both sides?

Metal height 5/8 and width 9/16

I can't identify on your website which balance matches the above?

Windows are about 11 years old. Don't know what the expected life of a balance is.

We can replace this balance, but it will have to be specially made. Before we can begin, can you just tell me how many you need? I assume you need 12 due to the fact you have 6 windows, but I wanted to confirm just in case.
