Thanks for posting. Start by looking at our S385-31 channel balance. I think this looks very similar to what you have, but let me know if you agree. It comes with a 19-005 top and 15-004 bottom nylon fitting.

Your old stamp of "30 5" usually matches up with our 3050 option, but you can weigh your sash on a scale and refer to the chart on our S385-31 store page to be 100% sure. Any common bathroom scale should do.
