Part is stamped Truth, 41011. I located it in the Truth catalog but without the flange rivited to the short arm, and without the glide rivited to the long arm. Can you provide a replacement complete with the added parts?

Your operator resembles our 39-253LB or 39-253RB, which uses the 39-529 track slider. For the brackets, see our 39-545 and 39-546. Look over the dimensions for each and compare them to yours to see if these items match what you need.
