Your Millard window crank reminds me of the 39-253RB Encore Dual Arm Operator. The 39-529 Truth Hardware Track Slider snaps onto the post at the end of the straight arm, which travels in the 39-528 track. You have the 39-545 Truth Snap-Stud Bracket connected to the link arm.

The options for replacing the cover and handle are the 39-376 and 39-452 Encore Cover and Handle Kits. The opposite handed versions of these kits can be found below along with the opposite operator and stud bracket.
