Mirror closet door pivot block

The closet door is 20 years old. The only label on it says Sunshine Mirror. No part number or any other info just that it meets safety standards.
The last photo shows the back or inside of the door. It was mounted in the bottom right corner.
I'd really appreciate any help you can give me.
