Hello, the mounting plate shown here is the part i'm looking for. The inset screws are 4 15/16", then the 2 screws for the handle itself are 5 3/4" part.
Please pay close attention to the last photo, "06717dc7-f6de-...", which shows the mounting plate from the side, so you can see the holes that are 4 15/16" apart also have side holes to screen in from the side of the door.

The plate you are searching for is not sold separately from the handle set. Your patio door handle set can likely be replaced with the 82-080 Mortise Style Handle Set. The keyed version is the 82-081.
