I'm afraid I can't say for sure what handle you need without seeing the original. The sticker tells me that you have a Silverline Series 5500 door, but this door has used many different styles. I've made a list of potential replacements below for you to review.
If none of these match, or if you aren't sure what's best, it would be best if you show me a photo of the part itself. We can then narrow down the list of possibilities considerably.
Here's a photo of the outside handle. The inside handle is broken close to the latch and not much to show.
Thanks for this photo, that's a huge help. What you have here is an older style handle that is discontinued and no longer made. My recommendation would be to replace the entire set (interior and exterior) with something like our 82-030. This should be fully compatible with your door and its existing hardware, but look it over and see what you think.
