For the channel balance, see either our S351-29 or S390-29. The only difference between these two balances is the depth of the bottom nylon shoe, or the 'heel-to-toe' measurement, so compare both with your old hardware to see what matches best. In either case you'll need a stamp option of 2840. For a new Top Sash Guide, see if the 18-177 matches up.
