Need Help On What Part I Need For Window #2

This is the 2nd window. I have attached 3 images of my window crank assembly and am hoping you can assist with identifying exactly what I need and the price.
The window hinges are on the left looking out the window.
The arm attaching to the crank is 9.5".

I will also need a couple of cranks and covers, but I am able to identify which of those that I need.

I hope I can help! This operator looks a lot like our 39-150 and 39-151. Carefully compare them to your own hardware and see how they match up.
