Need Part # for Sash Guide

Hello, I am trying to replace some sash guides that are about 18 years old, light yellow plastic. I have attached three photos. The height measures 1.5 inches, the width from the front is 9/16 inches, and the width from the side is 7/16" inches. Distance from the top of the guide to the edge of the second hole is 7/8 inches. The holes are rectangles with rounded edges, not circles. Hope you can help me with this one. Thanks! John
