Take a look at the multi-point locks related on this page compared to the one you're looking to replace. The hole spacing and projection of the locking fork are especially important.

As for the operator, see the 39-220B below, along with the 39-221B opposite-handed version. Review the specifications below to confirm it's a match and how the end of the link arm attaches to the sash.
