Can you help me identify the parts needed? Swivel balance, etc.

Thanks for posting. I believe this balance will have to be custom made. Before we begin, can you tell me the length of the outer tube? Can you also tell me how many you need? As for the bottom sash clips, see our 17-003. The nylon top guide is unfortunately no longer available. This window is very old and the company who made it went out of business.

The tubes are 16.5" and 28.5"

We have 3 windows that use the smaller size and 2 windows that use the larger size

Great! That's a huge help. Those lengths are a little atypical, but I think you'd be okay rounding up to 17" and 29". I'll email you custom order links for 6 of the 17" and 4 of the 29", including price quotes.
