Need help finding T-Lock Shoe

I have 5 windows that have broken pieces that connect to the balance spring, they have two set of numbers on them:
PAT 4610108
PAT 6,679,000

The balance spring they connect to has the numbers 32-5 on it and is 33 inches long, not sure if it's sold as a whole set.

Thanks for posting. This T-Lock Shoe looks like it might be our 15-195B 1-1/4" wide Open Cam shoe. Just look over the pictures listed on the product page to confirm this. Let me know what you think.
