I don't believe this old style operator is still available, unfortunately, but I picked two from our inventory for you to take a look at. 

The 39-194 that has one hole at the top of the housing and the two bottom hole locations at 1-3/8" (on center.) The other operator is the 39-411, with two holes at the top and with bottom hole centers at 1-1/2". Take a look at these two operators to see if either would work for your application.
