Need help identifying model number of plastic operator cover

Hello,
I'm having trouble identifying the manufacturer and model number and finding a replacement for operator cover shown in the attached photos. I cannot see any branding on the cover, and I only see a single number "3" in the center.

Hello! I'm not sure who the manufacturer is, since this cover is used on a couple different window brands, but I think our 39-107 is a good place to start. Look it over and let me know if you agree.
