Your pivot shoe resembles our 15-169. Take a look at the dimensions shown on our product page to confirm they match your specifications.

The balance assembly would need to be custom-made using the S770-34 (34D) and attaching the 15-170 clip to the end of the pulley cord. Before we send over your order link and price quote, please confirm that your metal channel measures exactly 34" long, as well as the number of balances you need with the minimum being two.
