Need help selecting channel balance

these balances are for Milgard vinyl non-tilt single hung windows purchased in 1999. Per image, the markings are for Caldwell balances 26 5 BS. Please note I have included images of both shoe and top for reference and have others available. They appear to be most closely matched to either the S380 or S385. Which one is correct? Channel length is 27"

Please note I just found the image on your website detailing the dimensional differences between the 15-004 and 15-005 shoes. I will measure to confirm but please also confirm that the winged tip is also correct.
