Looks like you have an old Acorn brand door. They're not around anymore, but I believe you can use our 82-055 as a substitute. This has a 3-1/2" installation hole center spacing, which was standard for Acorn doors. Look it over and let me know if you agree.

You may also want to check the condition of your tracks and rollers. The kind of damage I see in your first photo is usually a symptom of a door that isn't rolling properly. Basically, you want to make sure the track has no significant dents or dings, or that there's no dirt or debris that may be clogging the works. What's happening is that something is making the door harder to open, so you're forced to apply more pressure to compensate. This extra pressure will break the handle eventually and will definitely shorten the lifespan of any new hardware you install. It's worth looking into.
